ALLEN COUNTY HEALTH DEPARTMENT-Allen County Combined Health District	419-228-4457
The Health Department has several divisions that offer a variety of services. Vital Statistics records and issues birth and death records. The Maternal/Child Health programs provide Pregnancy testing and Medicaid application assistance for pregnant women. Health Department nurses coordinate Allen County's Bureau for Children with Medical Handicaps (BCMH). Pregnancy testing is available on Wednesdays and Fridays 8-9am and through the Family Planning Program (419-228-6154). Nurses also oversee Communicable Disease Control which includes general communicable disease and sexually transmitted diseases (STD). An STD clinic is available on Tuesdays 4:00-5:00pm. Tuberculosis (T.B.) testing is available Monday, Tuesday, Wednesday, & Friday, 8-9am and 3-4pm. Immunization Clinics for children are held every Wednesday, 1:30-3:30pm, Thursday 8:30-10:30am, with satellite locations at various times/locations around the county*. Nutritional counseling and supplemental foods are provided through the WIC Program (419-224-8200). Public health education is provided in the areas of AIDS, heart disease, nutrition, physical activity, and the health ministries program. HIV/AIDS testing is available by appointment. The Environmental Health Division regulates restaurants and swimming pools, provides sewage and plumbing inspections & issues well permits. The Environmental Division also provides follow-up for animal bites and public health complaints. Breast and Cervical Cancer Screening provided through Women's Preventive Healthcare Project, and community agencies. They also provide physician and community education. Diversion Department--provides diversion services and counseling for unruly and some delinquent youth (e.g., truants, runaways) and their parents designed to eliminate the need for formal court intervention. Probation Department--official supervision of adjudicated unruly and delinquent offenders within the community, Detention Center--provides temporary secure care for children pending court adjudication or disposition, Treatment Center--provides treatment, individual, family, group, and mental health counseling, and educational services for children in a secure setting after a court adjudication of delinquency, Day Treatment Program provides treatment, counseling and educational services to children in a non-secure setting after court adjudication, Restitution Program-provides a work-based program to permit adjudicated youth an opportunity to pay their court-ordered victim restitution, Community Services Program-provides a work-based program to permit adjudicated youth an opportunity to give service to their community as a form of rehabilitation.

Pregnancy counseling assists parents struggling with an unplanned pregnancy to make responsible decisions for the permanent care of their child. Help is given to develop the skills necessary for healthy beginnings in parenting the child or assistance for parents who choose to make an adoption plan for their child. With adoption decisions the birth parents are involved in selecting the right home for their child. A full range of pre adoption and post adoption services are provided to couples wanting to adopt and for families who have adopted. Services include domenstic and international home studies, education and training and post placement and supervision of adoptive placements. Adoption is a life long process which Catholic Charities prepares families for and supports them through out life. Search and reunion services are available for birth parents and adopted persons. Catholic Charitites - Diocese of Toledo Family Connections Program is licensed by the Ohio Department of Job and Family Services to license homes and place infants into homes.

The Child Health Clinic offers a variety of services to children who do not have a family doctor. Services include: Well Child Screening, age 0-21; Screenings for lead, sickle cell and low blood iron; Growth and Nutritional Assessment; Developmental Screening; Vision and Hearing Screening; Immunizations checked and updated; Health Education; Early discharge and 2 week check-ups for new babies; Referrals to Specialists when appropriate. Social services available include: Psychological Assessment, Screening, Referrals, and limited Counseling.

COMMUNITY CONNECTION FOR OHIO OFFENDERS, INC., DIVISION OF ALVIS HOUSE	419-225-8793
Community Services-Post Release Services: Community Connection offers support, assistance, consultations, transitional and educational programming and referrals for offenders, ex-offenders and their families. Services include: case management, employment assistance, information and referral services, and other assistance on an as needed basis. Insititutional Services-Aunt Mary's Storybook Project: Incarcerated parents tape record books for their children to strengthen family bonds and encourage literacy. Currently established in 12 Ohio prisons. Prison Literacy: Trained inmate tutors teach inmate students to increase their reading skills. Using the newspaper as a teaching tool, students gain basic life-skills and increase their employability. Restorative Justice:Community Connection addresses repairing the harm done to individuals by providing: a)victim awareness panels to offenders, b)the opportunity for victims of violent crimes to dialogue with their offenders, and c)ex-offender speakers to address inmates concerning personal accountability. We work to bridge the gap between victims and offenders in a constructive manner.

FAMILY PLANNING CENTER	419-228-6154
The Family Planning Center provides: reversible forms of birth control, education, and literature; pregnancy testing and referrals for prenatal care. Testing is available for sexually transmitted diseases, as well as pap smears, pelvic examinations, breast examinations and treatment for vaginitis. Other services include emergency contraception, referrals for sterilization for both men and women, and post abortion check-ups. The Family PLanning Center provides speakers and literature for public education and outreach. The Family Planning Center also provides clinical experiences for student nurses, sexual abuse nurse examiners, and nurse practitioners. HIV testing is available the fourth Monday of each month from 1:30-3:30pm.

Education and support group for families with brain disorders. Family to Family Education - A 12 week course that helps family members learn to cope with a loved one who has a brain disorder. The gruop focuses on education, medications, communication and support for the caregiver. The course is offed once a year in September.
