| The Health Department has several divisions that offer a variety of services. Vital Statistics records and issues birth and death records. The Maternal/Child Health programs provide Pregnancy testing and Medicaid application assistance for pregnant women. Health Department nurses coordinate Allen County's Bureau for Children with Medical Handicaps (BCMH). Pregnancy testing is available on Wednesdays and Fridays 8-9am and through the Family Planning Program (419-228-6154). Nurses also oversee Communicable Disease Control which includes general communicable disease and sexually transmitted diseases (STD). An STD clinic is available on Tuesdays 4:00-5:00pm. Tuberculosis (T.B.) testing is available Monday, Tuesday, Wednesday, & Friday, 8-9am and 3-4pm. Immunization Clinics for children are held every Wednesday, 1:30-3:30pm, Thursday 8:30-10:30am, with satellite locations at various times/locations around the county*. Nutritional counseling and supplemental foods are provided through the WIC Program (419-224-8200). Public health education is provided in the areas of AIDS, heart disease, nutrition, physical activity, and the health ministries program. HIV/AIDS testing is available by appointment. The Environmental Health Division regulates restaurants and swimming pools, provides sewage and plumbing inspections & issues well permits. The Environmental Division also provides follow-up for animal bites and public health complaints. Breast and Cervical Cancer Screening provided through Women's Preventive Healthcare Project, and community agencies. They also provide physician and community education. Business Skills Training – Teaches computer skills (Microsoft Word, Excel, PowerPoint, Access, and Mavis Beacon) and customer service training. 4. Janitorial Training – Assists individuals of obtaining janitorial skills through a classroom setting to assist individuals in the community who are either a displaced worker, convicted felon, or any individual having a hard time finding employment in the community. |

| 1. Vocational Evaluation/Assessment--Through a series of work samples and tests, Consumer's strenghts and interest are assessed. Provides a comprehensive evaluation of consumer's potential and setting of realistic vocational goal.
2. Work Adjustment Training--Consumers learn basic work skills necessary for maintaining successful employment in the competitive job market.
3. Job Coaching--Job coach provides one-to-one attention to get client started in employmnet. Goal is for consumers to function independently.
4. Job Development-This program assists the individual in attaining community employment based on skills and interest.
5. Work Services--Consumers work in a sheltered environment at their own pace. Each program is individualized with the goal to place consumers in competitive jobs.
6. Retail Sales Outlets--Goodwill operates retail stores in Lima (Eastside, 2314 Harding Road, 8AM to 9PM; Kenton (920 E. Columbus Street, 8AM to 9PM), St Mary's(212 E. Spring Street, 8AM to 9PM) and Van Wert (1220 S. Shannon Street, 9AM to 7PM). All stores are open Monday-Sunday.

8. Good Rides-Transportation services to and from work for persons who have barriers to tranportation.

| OHIO YOUTH ADVOCATE PROGRAM |

| The Northwest Region of Ohio Youth Advocate Program provides therapeutic foster care. Some of the programs in addition to the therapeutic foster care program are a CANEI (Constant and Never Ending Improvement)program. CANEI is a restorative justice program for adolescents with histories of aggressive defiant behavior. In addition, we provide respite and emergency homes in the Allen County area. Ohio Youth Advocate Program works with youth that have had multiple placements and have experienced abuse and neglect. The youth in OYAP care are the more difficult to place and the hardest to maintain in their placements. Ohio Youth Advocate Program provides mental health services which include CSP, diagnostic assessments and ongoing support for the youth and the foster parent. OYAP provides support for the youth and the foster parent. OYAP provides adoption services and, if necessary, adoption updates.
